This form provides a comprehensive overview of your health history and current symptoms. It includes questions related to digestion, hormones, inflammation, energy, stress, and overall patterns that may be influencing how you’re feeling.

Your responses allow me to identify key areas of focus and prepare for our work together with clarity and intention.

Food Journal

The food journal helps me understand your daily eating patterns, meal timing, and how your body responds to food. This information supports more precise guidance around digestion, blood sugar balance, inflammation, and energy.

The goal is to capture what’s typical, not perfect.  The food journal allows me to see what’s working well and identify areas where adjustments may be helpful.
